RESUMO

STUDY DESIGN: A retrospective cohort review. OBJECTIVES: The objective of this study was to investigate the efficacy of liposomal bupivacaine (LB) in patients undergoing lumbar spinal fusion. SUMMARY OF BACKGROUND DATA: Historically, posterior spinal fusion has been recognized as a particularly painful surgery. Postoperative pain limits early patient mobilization and discharge, and negatively impacts patient satisfaction. Local infiltration of anesthetic agents combined with postoperative multimodal pain management is common. On the basis of existing data, the liposomal formulation of bupivacaine might play a role in promoting faster recovery during the immediate postoperative period. The purpose of this study was to investigate the potential impact of LB on postoperative opioid requirements, ambulation, and duration of hospital stay, as well as potential health care cost savings. MATERIALS AND METHODS: A historical cohort of adult lumbar spinal fusion patients was retrospectively evaluated, in which 105 patients received nonliposomal anesthetic and 105 received LB. Both groups were managed with a standardized postoperative analgesia regimen. Demographic information, opioid consumption, length of stay, distance ambulated, and total cost of inpatient stay were collected. RESULTS: Although there was no difference in the pain scores between the 2 groups, the LB group was associated with significantly lower opioid usage throughout the postoperative period. More patients in the LB group were discharged within 2 days of surgery compared with the control group (88.6% vs. 38.1%, P<0.05). The control group was able to walk for a longer median distances (175 vs. 150 ft, P=0.02) on the first attempt, however, a significantly larger proportion of the LB group walked within the first 12 hours after surgery (61% vs. 3%, P<0.001). Also, LB usage was associated with $218 higher pharmacological cost compared with the control group but an overall $3035 lower cost for the entire hospitalization (P<0.001). CONCLUSION: Adjunctive usage of LB with lumbar fusion surgeries promotes earlier mobility, lower opioid consumption, and shorter length of stay resulting in overall lower health care cost. STUDY DESIGN: A retrospective analysis of the morbidity and mortality data collected by the SRS in 2009 with comparison to previous years. OBJECTIVES: Objective of this study was to assess the new format of morbidity and mortality data collection by the SRS in 2009 and evaluate the data collected with comparison to previous years. SUMMARY OF BACKGROUND DATA: 2009 morbidity and mortality reporting format was dramatically changed from previous years. This was done in an attempt to simplify the reporting process and to narrow the reporting to only three sentinel events: death, blindness, and neurologic injury. Only deformity cases including scoliosis, kyphosis, and Grade III or greater spondylolisthesis were included. METHODS: Results were obtained from the SRS M & M reporting summary for 2009. These included detailed analysis for complications including death, blindness, and neurologic injury. These were compared to similar statistics obtained from the years dating back to 2001. Blindness has not previously been reported. RESULTS: 80.1% of SRS members submitted 35,267 deformity cases. Both the percentage of members submitting data and the number of deformity cases far exceeded any previous year total. 57.6% of cases involved scoliosis, 10.5% kyphosis, and 31.9% spondylolisthesis. Neurologic complications were lower in each major category (scoliosis, kyphosis, and spondylolisthesis) compared to previous years. The death rate was similar to prior reporting 0.12%. Three cases of blindness were reported, two occurred in AIS. All three resolved. CONCLUSIONS: The altered format and requirement for all members to participate has dramatically increased the total number of deformity cases reported and percent of membership responding. Neurologic injury rates are decreased from previous years. Three cases of blindness occurred, all of which resolved. Further collection of data is needed to elucidate mechanism and prevention.

STUDY DESIGN: Retrospective review of the results of somatosensory evoked potentials (SSEP) performed in cervical spine surgery. PURPOSE: To evaluate the utility of spinal cord monitoring during cervical spine surgery in a single surgeon's practice, based on how often it prompted an intraoperative intervention. OVERVIEW OF LITERATURE: Intraoperative monitoring during cervical spine surgery is not a universally accepted standard of care. This is due in part to the paucity of literature regarding the impact of monitoring on patient management or outcome. METHODS: SSEP for tibial, median, and ulnar nerves were monitored in 809 consecutive cervical spine operations performed by a single surgeon. The average patient age was 52 years (range, 2 to 88 years), with 472 males and 339 females. Cases were screened for significant degradation or loss of SSEP data. Specific attention was paid to 1) what interventions were performed in response to the SSEP degradation with subsequent improvement, and 2) whether SSEP changes corresponded with postoperative neurological deficits. RESULTS: Seventeen of 809 patients (2.1%) had SSEP degradation that met warning criteria and therefore prompted intervention. Release of shoulder tape (8) or traction (4) most often resulted in SSEP improvement. Failure of SSEP data to return to within acceptable limits of baseline was associated with neurological deficit (p=0.04). Two patients awoke with new postoperative neurological deficits, which resolved in 6 hours and 2 months respectively. Patients with ossification of the posterior longitudinal ligament (OPLL) were at seven-fold greater risk of intraoperative SSEP degradation. CONCLUSIONS: SSEP monitoring in this surgical population proved sensitive to perioperative factors which may increase the risk of postoperative neurologic deficit, and probably prevented neurological deficits in 15 of 809 patients (1.9%). Improvement in data following intervention appears to correlate well with unchanged neurologic status. Experience with intraoperative monitoring in this patient series has led to incorporation of these techniques as a standard of care in cervical spine surgeries performed by this surgeon.

PURPOSE: To determine and quantify the relationship of osteoarthritis (OA) in the trapeziometacarpal, scaphotrapezial, and scaphotrapezoidal joints; to ascertain the dependability of radiographic assessment of trapeziometacarpal, scaphotrapezial, and scaphotrapezoidal OA; to determine the articular topography of the scaphotrapezio-trapezoidal (STT) joint (composed of the scaphotrapezial and scaphotrapezoidal articulations) using stereophotogrammetry; and to characterize the articular wear patterns of STT OA. METHODS: Sixty-nine fresh-frozen human cadaveric hands were staged radiographically and by gross visual examination for the presence of OA in the trapeziometacarpal and STT joints. Twenty randomly selected joints also were evaluated to determine the topography of the STT joint using stereophotogrammetry. RESULTS: Concomitant severe osteoarthritic degeneration was found in the trapeziometacarpal and STT joint in 60% of our specimens. A correlation was found in the severity of OA in the trapeziometacarpal and STT joints. Radiographic and gross visual evaluation of STT OA concurred in 39% of our specimens. CONCLUSIONS: The prevalence of concomitant trapeziometacarpal and STT OA, and the uncertainty of radiographic evaluation of STT OA, indicate the need to visualize the STT joint intraoperatively to determine the true degree of degenerative changes present.
